Though it’s a little hard to understand the question since I can’t see the poem in a whole, I’d say the author is describing a person, possibly old or sick, reaching for a glass of water. The language they’re using is a simile.

DAVID SMITH is an American sculptor(1906– 1965), he created metal figures that have affinities with the Abstract Expressionist movement in painting.
