Marty went to the doctor's office with a sore throat and an upset stomach. The doctor performed an exam and evaluation of Marty.
Marysya12 [62]

Answer:

- 25 modifier

Explanation:

The modifier 25 was used in this case because the doctor reported either a surgical procedure, X-rays, labs and supply codes that and this shows that it was documented as an E and M service that was separately identified but performed on the same day as though it is another procedure. Also, in a case where a patient visit the healthcare and it was not anticipated that a procedure might be carried out on her, but eventually it was, the E&M can also be assigned with the 25 modifier.
