This is a tricky question but I will try to help.

Answer A. gives off a sense of distance between the two characters and adds more importance onto what she says because of it.

Answer B. if it is referencing the enormous brown patch as the man who was transformed into a roach, seems more creepy and observational than tense. Just for that reason we can leave this one out.

Answer C. doesn't give off tension as much as it does anger. Therefore I don't think it would apply so we can cross that one out.

Answer D. carries with it the suspense of impending death and that implication makes the situation tense. Answer A. could also apply, but this answer seems to carry weight due to the possibility of death involved.
