The patient may fell that he or she is not valued and hence lose trust and confidence in the doctor. This is why doctors should be extra careful when writing notes.

Due to the sensitive matter of medical reports, choice of words is important so as to avoid emotional breakdown or even shock.

Another step is to ensure that the language used is easy to understand and information is well explained. This is to avoid any misunderstandings. Last but not least the choice of words should be right.
