Answer:

The author repeats "they came for" and "I did not speak out", which contributed its overall meaning because it shows that the speaker didn't care. The author didn't care every time they come for someone else. Over and over again the author did nothing. "I did not speak out" is repeated to show the many time the author did nothing. But then after the many times of doing nothing the author is in need of help but there is no one to help them because "they came for" everyone else and the author did nothing so now they weren't there for them.
