I would just walk away and say no, there are plenty of other healthy things I can do rather than just smoke a cigar. She might get mad at me be plenty of bad outcomes could come of it and I'd rather stay healthy and safe.

Rejecting a cigar is the best option you could do if you do not want to do it. Walking away from it is something that could help your dignity you give to yourself and if friends can not accept that they aren't really friends. Health is first then the rest to come.
