Just put something you are interested in, I'm interested in astrophysics and astronomy, so I would put that in some type of sentence.

What is something you are very good at

Just put a name you prefer, or your name
5. What will a successful school year look and feel like at the end of the year?
Put what you would like to accomplish this school year, I'm guessing art related since it's art class, but if you can add other things, maybe add things related to grades improving or something (not saying you have bad grades, that's just an example).
6. What are the characteristics or attributes you want in a teacher?
Put something like a characteristic that you would want a teacher to have, that might help you understand things better, or that will just make the classroom more joyful.
