Writers usually gain inspiration for their works by other books, shows, movies, or sometimes experiences they/others had. They begin to form their ideas usually by constructing how they want their book/story to progress. They write their ideas down, conflicts, characters, plots, etc. They decide what to say by writing a few examples, such as what POV it will be in, and what the first part/chapter will be about.
