The disorder to be suspected is Phenylketonuria.

Phenylketonuria is a genetic disorder. Its diagnosis can be done by identifying symptoms like musty odor emanating from body. So the nurse should suspect the same when such a symptom is exhibited by the child. This is a condition in which an amino acid known as phenylalanine is build up in the body. It is caused because of the absence of enzymes to convert this amino acid.

Other symptoms include  seizures, tremors, eczema, hyperactivity, stunted growth, etc. Early diagnosis is needed to avoid Phenylketonuria to get severe and cause damage to brain.
