The answer is B i believe. 

Your mindset is related to self confidence, because if you don't have the confidence to do something, and your telling your self you can't, you probably won't, because you have the mindset that you aren't good enough.

Attitude is related to mindset because If you go into something with a bad attitude, you have already made up your mind that you don't like something, and whatever your attitude is reflects your mindset.

Effort is related to mindset, because if you care about something or put alot of effort in something, you have a mindset that you will try your best, and you will.

Intelligence has nothing to do with mindset. YOu could be the smartest person in the world, but you can still have the same mindset as the dumbest person in the world if you have the same attitude, confidence, and effort.

I believe it's B, intelligence. 
A mindset has no correlation with whether you're smart or not. As long as you put in effort, and have a good attitude (which goes hand in hand with self-confidence) it'll put you in a good mindset. 
Being less smart than another person doesn't mean you have any less of a good mindset.
