Choice C is incorrect because there are not quotation marks present starting at the word, "Help," and ending at, "leg."

Choice D is incorrect because the comma is outside of the quotations between, "leg," and "screamed."

Thus, that leaves us with Choice B. Which is correct.

I believe the answer is D because it uses a comma, allowing the word 'screamed' to be lowercased.

I think it's C. Heavy means the sigh was long, and obviously loud. Wind through a tomb is used as a metaphor that the sigh was lonely. Lonely means nobody is there, and nobody is there to hear wind go though a tomb, and anyone in the tomb can't hear, because, well, they're dead.
