1)  The student may be unaware of the TRUE, UNDERLYING reason why s/he "misbehaved".

2)  The student may provide a response that is not a response that the [asker of the question] would be pleased to hear.  (e.g. "because I hate [insert name of the person].  ; or, "because that lesson is boring"; or, "because I'm sick and tired of school."

A client who has trouble falling asleep and remaining asleep would be diagnosed as having altered rest and comfort. If a client is having hallucinations or delusions, the diagnosis would be disturbed cognitive process.

For a person with attention deficit disorder or attention deficit hyperactivity disorder, ineffective impulse control would be the norm. An immediate illustration of a problem with sensory perception is the nursing diagnosis of acute confusion. Although it is not a direct example, sleep problems can affect sensory perception. Changes in sensory perception are what cause the sense of helplessness and inadequate coping.
