It is used to make sure you're in good health.

The top of it is mental, which means you need to stay healthy mentally.
The left corner is social, to say that you need to strive socially, and have a social life with your peers.
The right corner is physical, meaning like exercise. You need to exercise frequently to lose any unneeded weight, and gain muscle.

The health triangle is a basic measure of a person's overall health. It highlights different parts of what makes a person healthy. The triangle is then used as a basis for everyone to refer to when referring to what makes a person healthy and what areas people need to focus on.

The health triangle includes your physical, social, and mental health. Physical included the health of your body. Do you get enough sleep, are you eating balanced meals, are you exercising enough each week?

Your social health includes your socialization with others. If you liked to stay home and away from others, your social health is out of balance.

Your mental health includes your stress management, your anxiety level, how well you cope with problems.

When you see a doctor, they use a basic health triangle to cover all aspects of health to see how healthy you are.
