The nurse should say the following sentence - "I'd like to understand what you're saying, but I'm having trouble following you."
She obviously cannot understand the man because he is saying things that have nothing to do with the conversation they are having. She should be polite and tell him that she cannot understand him, even though she would like to. Maybe he will manage to be clear once he takes his time.
