This sentence is in ACTIVE VOICE. Active voice is when the subject (the quizzes) performs the action. In this case, the quizzes cause failure of the students.

In passive voice, the subject of the sentence is acted on by the verb. 
In the examples above: All...were failed, students...were injured, class...was filled. To put those sentences in active voice you might say:The students failed the chemistry test, Chemicals injured the students, and Students filled the chemistry class.</span>
