Where?) The three mile island was in dauphin county, Pennsylvania. 

What?) It was the most significant accident that happened in the U.S commercial nuclear power plant history.

Happened?) It began when there was failures of the non-nuclear secondary system. Which led to large amounts of nuclear reactant coolant to escape, causing people to die and led to cancer (etc.) <span>ʕ•ᴥ•ʔ</span>
