The sentence in the active voice is: "In each inning, the pitcher threw at least one strike".  In the passive voice, the active subject, i.e. the subject that performs the action, precedes the verb. Tenses formed with the auxiliary verb "to be" and the past participle, followed by the preposition "by" when the performer of the action is explicitly mentioned, are not used in the active voice.
