It takes the help of information and technology to move towards a Community Health Record.

Explanation:

The use of technology by the stake holders of community health of multisector to aggregate the information and to use it to understand the health of a community is referred as Community Health Record model.  

This model is used as a guideline of public health, health care, and collaboration of community and exchange of health information between them.  It has the benefit to its users of working as an aid to the health development of a community. One important drawback of this system is its financial issues.
