First of all, it's not easy for people who are in other countries to get into his country. Also, it would be very hard for people to try to kill him. The people under Kim don't rise up. Probably the main reason is Kim has this web of people around him to protect him so whoever would be brave eough to do it would probably be killed and their family would be killed. This makes people afraid.
