me need slope and y intercept to get it in y=mx+b form (slope intercept)

Slope is the

difference in y / differences in x

differences in y = 6.5 -3  = 3.5  

difference in x = 2 - 1 = 1

so slope is 3.5/1 or just 3.5

(now you can see the answer is c but lets check it by getting the y intercept too)
