Processing Claims

A number of technical protocols and industry standards must be met for insurance claims to be delivered expediently and accurately between medical practice and payer.

Medical billing specialists typically use software to record patient data, prepare claims, and submit them to the appropriate party, but there isn’t a universal software application that all healthcare providers and insurance companies use. Even so, insurance claims software use a set of standards, mandated as by the HIPAA Transactions and Code Set Rule (TCS). Adopted in 2003, the TCS is defined by the Accredited Standards Committee (ACS X12), which is a body tasked with standardizing electronic information exchanges in the healthcare industry.

There are two different methods used to deliver insurance claims to the payer: manually (on paper) and electronically. The majority of healthcare providers and insurance companies prefer electronic claim systems. They are faster, more accurate, and are cheaper to process (electronic systems save around $3 per claim). But because paper claims have not yet been completely removed from the insurance claims process, it is important for the medical biller and coder to be well versed with both electronic and hardcopy claims.

Filing Electronic Claims

Certain technologies have been introduced into the system in order to expedite claim processing and increase accuracy.

Software

Some healthcare providers use software to electronically enter information into CMS-1500 and UB-04 documents. Using “fill and print” software eliminates the possibility for unreadable information. This software may also include certain types of “scrubbing,” or tools that check for errors in the documents. While these tools do decrease the amount of errors made in filling out claim forms, they are not always 100 percent accurate, so medical billers should remain diligent when filling out forms using software.

OFFERING ALL MY POINTS AND BRAINLIEST!! NO JOKING, I NEED IT DONE ASAP!

Answer:

Explanation:

Many people want a cereal with the most fiber possible and/or wholegrains, but with the lowest amount of sugars and sodium. Cereals are one of the easiest ways to boost your fiber intake. For this the product I selected is All-Bran. This cereal is very good since it contains a lot of fiber. Many of the children from the age of 6 to 12 consume milk with cereal and together they lead to a source of 10 essential vitamins and minerals. A bowl of bran cereal gives you 9 grams of fiber which is around one-third of your recommended daily intake. This cereal contains between 15 to 30 percent of fiber while other cereals have less or even though they may have nothing. The cereal contains 13.3 grams of fiber per serving in a 45 gram serve size.

The ingredients of all-bran cereal are: Wheat bran, sugar, malt flavor, contains 2% or less of salt. Vitamins and Minerals: Calcium carbonate, vitamin C (sodium ascorbate and ascorbic acid), reduced iron, niacinamide, vitamin B6 (pyridoxine hydrochloride), vitamin B2 (riboflavin), folic acid, vitamin B1 (thiamin hydrochloride), vitamin A palmitate, vitamin B12, vitamin D. In a serving size of ½ cup, this product contains 80 calories in which it contains one gram of fat, 80 milligrams of sodium, 6 grams of sugar, and 10 grams of dietary fiber.

Some benefits that we can provide from fiber is that soluble fiber dissolves into a gel-like substance during the digestive process.  When it is moving through the digestive tract, it picks up LDL cholesterol molecules and helps remove them from the body, which is great because the bad cholesterol accumulates along blood vessel walls and blocking normal blood flow which can also increase the risk of cardiovascular disease. Even though fiber has numerous amounts of benefits, everything in excess is bad, and having an excess of fiber and eating too much can lead to uncomfortable side effects and potentially serious complications. Since fiber is not actually digested by the body, the digestive tract has a slightly more difficult time moving it through the system.  So if you fill it with too much fiber, you can experience some problems as cramping, bloating, gas and abdominal discomfort.

Other product I selected is Honey Nut Cheerios cereal. This cereal contains 12 vitamins and minerals which are good for your body; it is also low in fat, it contains a good source of calcium, it is gluten free, and it is made with whole grain. Each “o” of this cereal contains soluble fiber from the whole grain oats. Eating this product is very good since it contains beta-glucan which is a soluble fiber proven to help lower cholesterol as part of a heart-healthy diet. This product also contains a lot of carbohydrates since it contains 80 grams of carbohydrates in a 100 gram package. Of those 80 grams of carbohydrates, 7 grams are fiber and 33 grams are sugars. In some point it is bad for your body because it contains more than 15% of the daily value that you can consume, and it only contains 2 grams of fiber.

The ingredients of that the Honey Nut cheerios contains are: Whole Grain Oats (Includes the Oat Bran), Sugar, Modified Corn Starch, Honey, Brown Sugar Syrup, Salt, Tripotassium Phosphate, Canola and/or Rice Bran Oil, Natural Almond Flavor, Vitamin E (Mixed Tocopherols) Added to Preserve Freshness. Vitamins and Minerals: Calcium Carbonate, Zinc and Iron (Mineral Nutrients), Vitamin C (Sodium Ascorbate), a B Vitamin (Niacinamide), Vitamin B6 (Pyridoxine Hydrochloride), Vitamin B2 (Riboflavin), Vitamin B1 (Thiamin Mononitrate), Vitamin A (Palmitate), a B Vitamin (Folic Acid), Vitamin B12, Vitamin D3.

With this information we can notice that one of the ingredients is sugar, not honey. Anyhow, the amount of sugar is of 2.5 tablespoons which is less than other children’s cereals. A substitute for this can be the regular Cheerios because it contains less sugar and more fiber. It doesn’t contain artificial colors neither trans-fats.

In my opinion, any plain cereal based on oats or wheat biscuits is great for a healthy breakfast. Some people like to add sugar or honey to sweet their meal. one important thing to mention when buying cereals is the price and the marketing it uses. In Cheerios, the bee cartoon catches the attention of little kids and the sensation or having a taste of honey is very good for them. Dietary fiber comes from plant foods, which often have both soluble and insoluble forms of fiber. Some of the best sources of soluble fiber include oats, beans, apples, carrots, barley and peas. Excellent sources of insoluble fiber include whole-wheat flour, potatoes, wheat bran, kidney beans, nuts, pears, corn and broccoli.  Another way to substitute cereal and obtain all the nutrients from them is by eating healthy; for example, fresh fruits, vegetables, etc.
