In a dialogue, it is necessary to mention the speech or dialogue that had been spoken out. and to do that, it is also necessary to use the quotation marks to denote which is the spoken words. So, whatever punctuation marks that the quote uses, be it a comma, period or question mark etc will go inside the quotation mark (" ").

For example,

He said, <u><em>"Why do you always have to make things complicated?"</em></u>

The punctuation marks in this line of dialogue is the question mark (?) which is kept inside the quotation mark (" ").
