  3. <u>You used an out-of-network provider:</u> The insurance company may deny to provide any services to the  client in case if he has approached an out of plan or out-of-network provider. As it will cause the conflict of interest resulting in not having any services been provided by the insurance company.
  4. <u>There are minor transcription errors:</u>Even, minor errors or having some issues in the data entered or the record saved with the insurance company can lead to more confusion and denial of the insurance to provide any services to its clients.But, it can be corrected well by the customer representatives when approached on time.
