Concussion doesn't require a direct blow to the head, but any strong force that bumps the brain strong enough, that the hit can affect brain's function. Most people suffering from concussion won't lose consciousness.
If the person is suffering a heavy head concussion, they should be immediately transferred to the doctor's office for the further evaluation, CT or MRI scan.
