I believe that A is the perfect pariticipial phrase of all the examples shown.  Participial phrases have a verb phrase in the participle form, either the present form (-ing) or the past one (-ed, -d).   "Having assesed the students' performance on the quiz", in my opinion, is a subordinate clause which funtions as a modifier of time of the main clause "Mrs. Bunch decided to review the causes of the Civil War". You could understand it by adding the PP "after": "After having assessed the (...)".  

In B "indicating" is the VP of the subordinate clause modifying the NP "a map". The verb "to indicate" selects an implicit subject and an explicit D.O (direct object), the NP "the major battles of the Civil War".

In C, the verb "to pose", in the -ing form, is the verb of the reduced relative clause modifying the NP "team members". That is, team members, who were posing as a soldiers,  re-enacted the Civil War events would be the complete sentence.

In D, the verb "to include" is in the past form. It is not a participle phrase, but a verb phrase, said transitive verb select an NP subject and a coordinated NP direct object.
