Don't mean to be rude but the correct grammatical way of that Sentence is "How can a mother give birth to two offspring (young)"
When the egg is fertilized by the sperm during some stages the fertilized egg can split into two  identical twins (look the same). And fraternal twins (dont look the same different genders) this is because with the male sperm two eggs are fertilized separately but grow in the same womb
