In the above sentence the past participle form of Present verb (cook) is (cooked), and it has been written after adverb (always).

(4) Then generally preposition ( by) will be written.

So in the above sentence preposition ( by) has been written.

(5) Finally the (subject) of active voice will become the (object )of passive voice and it will place at the end of passive voice sentence after preposition ( by).

In the above active voice sentence the subject is (she) ,and when it will be used as (object) in the sentence, instead of the word (she) it will be written as (her) .

We. Know ( her) is the object of the subject (she).

So in the above passive voice the subject of active voice ( she) has been written as ( her) in the object of passive voice sentence.

Thus the above Passive Voice answer, (delicious food is always cooked by her), has been written by using the above rule for changing active to passive voice of present Indefinite tense.
