The addition of an equal amount of water could have caused the blood solution to become hypotonic that is with low salt concentration and high water potential.

Since the solution is hypotonic and the concentration of water is high in blood than in blood cell, the water will move inside the Red blood cell through diffusion and the cell will swell and could burst. This would lead to the death of the patient.

Thus, the blood sample will become hypotonic as a result of which RBC will swell is the correct answer.
