If the plasma membrane is permeable to sodium (this means that sodium particles can go through the membrane), there will be a net movement of sodium from the inside of the cell to the outside of the cell, because there is a tendency to equate the concentrations between the intracellular space and the extracellular space - this process is called diffusion and it's generated when there's a difference (gradient) in the concentration of a particle between two sides that are separated by a permeable membrane.

If the plasma membrane is not permeable to sodium, sodium particles will not be able to diffuse through the membrane. Instead, the gradient in concentration will make the water move from the place with less sodium concentration (extracellular space) to the place with the higher sodium concentration (intracellular space), again to equal the concentrations between the two sides - this process is called osmosis.
