Michelle finally landed her first job after college and it provides insurance. She had surgery on her knee that she was told wou

ld be covered by her insurance, but now she has received a bill from the hospital saying the insurance did not pay it. She called the insurance company and they said it has been paid, but the hospital billing department tells her they never received payment.
