Very few percentage of people in developed countries are able to afford the cost of implants as majority. Most of the earnings of people in developed countries are spent on basic necessities of life like food, clothing and shelter.
Underdeveloped countries have people with income issues they will have the less amount of money.
High cost and majority of implanting were done in under developed countries is absurd because as said earlier underdeveloped countries cannot afford costly treatments.
