We can see this through the usage of the word 'patient' in the excerpt which indicates there is something wrong about him and this part of this sentence : </span><span><em>were permitted </em><span><em>to roam about the house </em></span></span><em>and grounds in the ordinary apparel of persons in right mind. . . </em>which indicates that he's losing his mind.
