If you carefully match each shape to it's sides, Net A is the correct one. The key is the 14 and 13 sides. The 14 should be vertical in the net.

Surface area is just the area of each individual shape together.

There are the following shapes and dimensions:
Rectangle: 5 x 14 = 70
Rectangle: 12 x 14 = 168
Rectangle: 13 x 14 = 182
2 Triangles: 1/2 x 5 x 12 = 30 x 2 = 60

70 + 168 + 182 + 60 = 480 square inches.
