Between the lines of Ophelia's hysterical delusion, it is possible to identify the nuances of what triggered her imbalance and, finally, death - that no one knows for sure if it was an accident or if Ophelia provoked the tragedy.  

For that reason, the correct answer is that she drowns, although the exact details of the death of Ophelia are still in debate.
