  • In articular problems, active and passive range of motion is restricted.
  • In non-articular problem, only articular range of motion is restricted.

The joints are formed by the articulation of the two or more bones and muscles. These joints are the characteristics of the range of motion. The articulation of the bone type determines the type of movement. The articular structure is consisting of synovial fluid, synovium, intra-articular ligaments, articular cartilage, and joint capsule. Bone, muscle, nerve, tendons, ligaments are non-articular structure.

  • Articular problems are described by swelling, diffuse pain and a very limited range of motion on passive and active movement.
  • Non-articular problems are more painful in the active range of motion, whereas no pain in passive motion.

To examine the active range of motion, a doctor asked a client to move their body. There is no external force or pressure is applied to the body, whereas in a passive range of motion, an external force is applied to move the body. In the case of a particular problem, there is a restriction in the movement of joints.

In the active range of motion, the test is done by asking the patient to move their body themselves. In this case, the patient should use their muscle to move their body. If there is a restriction in this examination, it could be caused by the muscles or the joints, or the nervous system that used to contract the muscle.
In the passive range of motion, the test is done by the examiner moving the patient body. Since no muscle used, the test shouldn't be influenced much by the muscle. Then, if there is an articular joint issue, the restriction would be found in both passive and active ROM, but the nonarticular joint issue might only cause abnormality in the active ROM.
