Government's failure to control the quality of water in Flint, Michigan.

Explanation:

In 2014, a health crisis occurred in flint, Michigan. They found that the water that they usually used as drinking source was constipated with Legionnaires' disease and dangerous amount of Lead.

Every states have their own budget to maintain the quality of the water. But, the government of Michigan opted to cut the cost of maintenance by not following the proper regulations. This what caused the decrease in water quality.
