Do doctors have to have orders to administer a medication

Not usually... Most MD's are quite capable at administering medications and drugs on their own, seeing as aside from the medical director, they have a more all encompassing scope of practice.  However some doctors may have some limits, ie approval from a patient, or a medication that is entirely within another doctors specialization entirely.
