Like for example, the questions asks "whats something that you did wrong or failed at and how did you handle it?"  You can say something like "I tried to bake a cake for my mothers birthday and I put the wrong ingredients in, and burnt it. I had to throw it away because of how burnt and disgusting it turned out." Then in the third box you can write: "I burnt the cake because I wasn't paying attention to the stove, and I was just mixing random ingrediants in that i felt would make a perfect cake. I resolved the situation by following a cake recipe, and setting a timer for the cake so i won't burn it." Something like that.
