The answer is:

y=1/5x+3

Because:

y=mx+b

m=slope
x stays the same
b= y intercept (which is what number the line hits the y axis, which is 3 in this case)

the slope (m) is 1/5 because we use rise/run, the slope rises 1 and runs 5 before it hits another point.
