The correct answer would be - prevent direct physical contact with hands and equipment.

Enterococci are bacteria that are naturally present in the intestinal tract of all people. Vancomycin-resistant strains of this bacteria are referred to as Vancomycin-Resistant Enterococci or VRE. If VRE infects the urinary tract, Injuries, or the bloodstream of hospitalized patients, can become serious to threatening.

It could be transmitted or spread by direct contact with hands, or medical equipment that has been contaminated.

Guidelines regarding handling infectious material and equipment:

 • The nurse should use disposable gloves and wash their hands with soap after caring for a person with VRE.
 • A disposable gown in case of washing or turning the patient, or changing diapers.
 • Using bleach or disinfectant to clean toilets and commodes.
 • Equipment should be assigned and used solely to the infected patient only.

Thus, the mentioned guidelines above should be used by the nurse for the VRE patient care.

The wound care nurse evaluates a client's wound after being consulted. the client's wound healing has been slow. upon assessment
Virty [35]

Answer:

Desiccation

Explanation:

Skin healing involves a huge range of events and special situations. It consists of a perfect and coordinated cascade of cellular and molecular events that interact for repaving and tissue reconstitution. With all the other metabolic processes in the body, healing depends largely on the degree of hydration the individual's body is. In cases of dehydration, desiccation may occur, which is localized dehydration of a wound, which consists of slow healing and scabbing of the wound tissues.
