(As a spanish speaker I can say that some phrases are not structured good, so I couldn't provide the answer for question #4... sorry)

Who is giving you these questions? They are very badly formulated. As a Spanish speaker, these are barely even understandable.

What I would do on question 4, is add “Te” in front of the verb. This makes it seem more like a question. “Te disfrazaste”, it means “Did you dress up”.
