Answer:

You could draw a character with thorn branches spewing from the nose and mouth, like the character is desperately trying to rip them out.

Another idea is a an image of a character looking into a mirror, and the mirror is showing a version of themselves they wish they were.

My last idea is a character in a room full of familiar people, but the character is cast aside.
