So, Sally's employer pay to his health premium will be,

38\%\times 4350=1653 dollar.

So, Sally has to pay the remaining amount (4350-1653=2697) dollars.

Since sally pays the remaining balance by having deduction in equal amounts from her 26 paychecks, So the amount deducted from each paycheck will be,

\dfrac{2697}{26}=103.73 .

Hence sally have deducted $103.73 from each paycheck.
