The standard form of a cosine function is \sf \: y = a \sin(bx - c)  + d Where A Amplitude (height), b is period, C is phase shift, D is vertical shift. We are given the Amplitude, Period and Midline. Midline means the same as phase shift. So, the function formed will be

2 = 5 \sin(\pi x - 2)  + 1
