Declaring that the client needs to take a shower right away or demanding that the staff provide the client a shower could make the client feel more anxious and helpless and exacerbate any delusional ideas they may be experiencing. It will not improve the client's self-image to gently point out that their appearance is disturbing the other clients, and it is irrelevant what the other clients may think.
