Before building enough trust to strip for a shower, the client must feel at ease in the setting; the nurse's comment gives the client the freedom to decide. Declaring that the client needs to take a shower right away or demanding that the staff provide the client a shower could make the client feel more anxious and helpless and exacerbate any delusional ideas they may be experiencing. It will not improve the client's self-image to gently point out that their appearance is disturbing the other clients, and it is irrelevant what the other clients may think.

There is a passage in the Bible that refers to Ham, the son of Noah, acting out against his father, and in return his skin gets blemished or darkened making him inferior and showing the crime he did against his father. This was the main excuse for enslavement of those with darker skin for generations, and if it was in their most sacred book, how could it possibly be wrong?
