Blunt abdominal injury is defined as damage to the abdomen and/or abdominal organs secondary to impact with a blunt (not penetrating) object or to alert patients free of diverting injuries, the most reliable symptoms and signs of Blunt abdominal injury are abdominal pain, abdominal tenderness, and peritoneal findings, especially when risk factors for abdominal injury are present. During blunt abdominal injury, the patient is lying down in a position of greatest comfort, usually on the back or on the uninjured side, with both knees worm up for the relief of pain and spasm. Loosen any tight clothing, especially at the waist and neck.

So we can conclude that indications for laparotomy in a patient with a blunt abdominal injury has signs of peritonitis, uncontrolled shock, or hemorrhage.
