I think that the physician and the medical assistant, Meredith, have an equal part of responsibility. Meredith because you are trained and know that you should always document the correct vital signs every time and for every patient, especially blood pressure. If she was having a problem getting a correct reading, she should have asked for help from another assistant in the office or have tried another sphygmomanometer. Also she did not review Mr. Wallace's file and I assume she didn't ask what was the cause of visit.

The permanent birth control method is vasectomy. A vasectomy is a procedure to make a man sterile, by blocking sperm from reaching the semen that is ejaculated from the penis. The fluid contains semen, but no sperm, so it prevents the pregnancy.

There are many birth control methods, some more effective than others. Some can prevent you from STD (Sexual Transmitted Disease) disease.  

1. Contraceptive pills/birth control pills

Birth control pill contains a combination of progesterone and estrogen hormone to control the ovaries and the uterus. Birth control pill works best when it is taken every single day at the same time of day. A Pill is an effective form of birth control, but even missing one day increases the chance of getting pregnant.

2. Male condoms.

This type of birth control not only prevent pregnancy but also prevent you from STD disease

3. The intrauterine device (IUD)

IUD is a coil and flexible T-shaped device that is placed in the uterus by a physician. There are two types of IUD, a copper, and a hormonal IUD.

4. Injections

It prevents pregnancy by stopping the woman from releasing an egg. The shot is injected every 3 months at a doctor's office.
