D)MRS. PETERS (starts to speak, glances up, where footsteps are heard in the room above. In a low voice.)

This stage direction most adds to the suspenseful tone. When Mrs. Peters pauses her dialogue to look up, it shows that something isn't quite right. After the footsteps are heard, Mrs. Peters continues speaking, but in a low voice. This indicates that what she is saying isn't meant to be heard by everyone. The secrecy creates a suspenseful tone. Merely speaking the other woman, shaking one's head, or arranging pans do not give the feeling of suspense.

MRS. PETERS (starts to speak, glances up, where footsteps are heard in the room above. In a low voice)

Further Explanation:

Trifles is a play that was written and performed by Susan Glaspell and this play was performed for the first time at the Wharf Theatre in Massachusetts. In this play, Glaspell played the role of Mrs. Hale and within no time this play gained popularity and was added to the American textbooks. This play is also followed up by the short story, “A Jury of Her Peers” which was also written by the same writer and also carries the same characters and plotline.

In the above-given phrase from the question, The stage direction mostly adds to the suspenseful tone and when Mrs. Peters pauses her ‘dialogue’ to look up, It also shows that something is not quite right and after the ‘footsteps’ are heard, Mrs. Peters continues ‘speaking’ but in a ‘low voice’. The reason she is saying in low voice is that she wants no one to hear their conversation and wants to keep the secrecy of the conversation.
