The nurse is assessing a 6-month-old infant prior to scheduled immunizations. what finding would the nurse immediately recognize
artcher [175]

The infant with fever should be assessed by nurse and immunisation administration should be rescheduled.

The main contraindication of most vaccines and toxoids is acute febrile illness. Fussy crying ,poor feeding and  loose stools would need  further assessment but will not contraindicate the administration of a vaccine.

Immunisation in patient with fever may cause:-

  • it may prolong the current illness
  • may cause immunisation reaction
  • It can cause adverse reaction
  • fever should be investigated further

The absolute contraindications applicable to all vaccines:

> anaphylaxis following a previous dose of  relevant vaccine

> anaphylaxis following any component of the relevant vaccine..

Immunisation should be done once fever or acute febrile illness is completely cured. Immunization is the process of giving a vaccine to a person to protect them against the disease.

What is an example of a decision-making framework?
Molodets [167]

One example of decision making frame work would be listing possible solutions. This stage is important to the overall decision making processes as a decision will be made from a selection of fixed choices. Always remember to consider the possibility of not making a decision or doing nothing and be aware that both options are actually potential solutions in themselves.

Hope this helps :D
