The last one is the good one.

The point slope formula is,

$y-y_0=m(x-x_0)$

Where,

$m=\frac{y_1-y_0}{x_1-x_0}$

And,

$(x_1,y_1)=(-3,-4)$

$(x_0,y_0)=(0,2)$

So, 

$y-(-4)=\frac{-4-2}{-3-0}(x-(-3))$

$y+4=2(x+3)$
