The point slope formula is,

y-y_0=m(x-x_0)

Where,

m=\frac{y_1-y_0}{x_1-x_0}

And,

(x_1,y_1)=(-3,-4)

(x_0,y_0)=(0,2)

So, 

y-(-4)=\frac{-4-2}{-3-0}(x-(-3))

y+4=2(x+3)
